Our annual membership supports a more personalized care model with direct portal access to your provider, longer appointment times, specialty testing options, discounted self-pay labs, and ongoing support throughout your wellness journey. It also helps cover the time spent outside visits reviewing labs, developing care plans, answering questions, managing refills, and continuing advanced education so your care remains thoughtful and individualized.

Add a spouse or adult child, ages 18–22 and living in the home, to your annual membership for an additional $120 per person per year.

Cancellations with less than 24 hours’ notice or same-day no-shows may incur an $85 cancellation fee. We understand emergencies happen, but we ask for advance notice whenever possible.

$30 insurance lab draw: out-of-pocket costs depend on your insurance copays and deductibles. We cannot guarantee that labs will be covered by your plan.
Other labs drawn throughout your care plan are charged at a fraction of the costs that are typically billed to insurance.
