Annual Membership
Our annual membership fee ensures that you have direct access to your provider through a patient portal, as well as access to specialty testing, discounted self-pay labs, and comprehensive support throughout your wellness journey. You can expect longer appointment times and much shorter turn around times for patient refill requests and answers to questions or concerns. This also allows the provider to be compensated for the many hours spent outside of the clinic to ensure comprehensive care plans are developed for each patient as well as ongoing advanced education.

Family Add on
Add on a spouse or an adult child (age 18-22) living in the home to your annual membership for an additional $120 per person per year.

Office Visits
$325 first visit: Covers annual membership fee + discounted office visit
$85 in-person/telehealth
$30 / InBody for non-member (free quarterly for members)
Cancellations with less than 24 hours notice or same day no shows may incur an $85 cancellation fee. While we understand that emergencies cannot be planned for, we do ask for advanced notice in non emergent scenarios.

$30 for insurance lab draw: out of pocket costs depend on your insurance copays/deductibles. We cannot guarantee that labs will be covered by your plan.
Other labs drawn throughout your care plan are charged at a fraction of the costs that are typically billed to insurance.
